NEXPLANON Training

Due to a recent label update, it is important to fully understand the updates to the insertion and removal procedures prior to inserting Nexplanon. If you were trained PRIOR to October 15, 2018, you are required to complete the mandatory training module on the updated insertion and removal procedures for Nexplanon. You will need your NPI number for initial log in and you will be prompted to create a user name and password. The training module will take approximately 25 minutes to complete. If you do not have your NPI number please email me directly and I can provide it you.

You must complete this module by May 1, 2019, in order to continue to order Nexplanon. If you do not complete this module, you will be required to attend an in-person training for re-certification for Nexplanon.

You will receive email confirmation within 3 business days of completion. Once you receive your confirmation please forward me a copy to file in your individual folder.


ZSFG New On-Call Parking Cards

These cards are specifically for on-call parking, which means for any and all shifts that start at or later than 6PM during the week or all weekend shifts. Abuses of the card will result in indefinite suspension of complimentary night-call parking privileges.

The card is to be returned up graduation along with all other identifications, cards, scrubs, etc. Failure to return the card will result in the resident being charged a $35 replacement fee. Should the resident lose the card at any time prior to that date, the resident will be charged a $35 replacement fee.

The LAZ parking systems is set up to provide 65 free parking spaces to residents per evening. If the resident attempts to use the card and is rejected, that means that the allotted spaces have been used for the night. This card provides for free on-call parking when available, but does not guarantee parking.

REIMBURSEMENT REMINDER

You need to submit your request for reimbursement immediately after your return from a trip. There is a new rule in place - If your reimbursement is not complete and submitted within 45 days of travel, the reimbursement amount will be taxed. This means that you will pay taxes on that amount.
